Educator – Professional Library Card Registration

The Educator – Professional library card registration forms needs to be completed in the library.

Educators and Other Professionals Loan Service Guidelines:

• Patrons must be 18 years of age or older to register for the Educator – Professional library card. 

• The Educator – Professional library card should be used for education and professional purposes only. Personal use of the Educator – Professional library card may lead to the cancellation of the cardholder’s privileges.

• Patron must have identification showing name and current ailing address to sign up for the Educator – Professional library card.

• Loan periods, fine rates and renewal limits will vary. Loan periods will be listed on the date due receipt. All regular circulation policies and procedures of each CLEVNET library apply to the use of this card.

• The Educator – Professional library card cannot be issued if a patron has $10.oo or more in fines./fees on their personal library card. Once fines/fees are below $10.00 an Educator – Professional can be issued.

• A maximum of 99 items may be checked out on this card. Materials may be renewable, depending on the item.

• The individual signing for the Educator – Professional library card is responsible for all transactions on this card.

• The card holder is responsible for all unreturned materials, overdue fees and lost/damaged materials.

• It is the responsibility of the card holder to notify the Wayne County Public Library if the card is misplaced or stolen. A new card will be issued for a minimal fee, and the original card number will be listed as invalid.

• The Educator – Professional library card may not be duplicated. If an Educator – Professional library card is found to be duplicated or photocopies, this could lead to the cancellation of the cardholder’s privileges.

• The Educator – Professional library card will expire in one year (365 days). All cards will be updated and renewed each year,. If the card is not used after a period of time it will be canceled.
